Pseudotumor kutaner inflammatorischer D48.5

Bahadori u. Liebow 1973; Erstbeschreibung in der Haut:  Hurt und Santa Cruz 1990;

Sehr seltenes (weltweit sind  nur wenige Fälle beschrieben), meist extrakutan (Lunge, Milz, Lymphknonten) lokalisiertes, gutartiges, plasmazellreiches (auch Plasmazellgranulom) Pseudolymphom.

Unbekannt. Diskutiert werden eine ausschließlich entzündliche Genese sowie infektiöse Ursachen, z.B. EBV-Infektion.

V.a. Lunge, Milz, Lymphknoten, Haut (selten) sind betroffen.

Klinisch typisch ist eine solitäre, asymptomatische, rot-braune Plaque oder ein entprechender Knoten mit glatter Oberfläche. Selten ist ein multiples Auftreten.  

Dermales knotenförmiges Infiltrat mit Lymphozyten, zahlreichen Plasmazellen und reaktiven Keimzentren (hier Expression von CD20; CD79a). Vereinzelt eosinophile und neutrophile Granulozyten. Tendenz zur Fibrosierung.

Immunhistologie: Polyklonalität des Infiltrates.

Plasmozytom, extranodales B-Zell-Lymphom vom MALT-Typ.

Exzision ist die Behandlung der Wahl.
