Simple clinical test which, together with the typical skin lesions and histological examination, leads to the diagnosis of cutaneous mastocytosis (e.g. urticaria pigmentosa or mastocytoma). If the lesions are rubbed or scratched, within a few seconds there is a localised, typical, strong redness and possibly urticarial swelling, possibly itching or, rarely, blistering (positive rubbing test). The Darier sign has a high diagnostic value for cutaneous mastocytosis.
